Our anonymous national reporting system (Meldeordningen) for errors in hospitals has until now been a unique source for learning, and for preventing errors from re-occurring. It is now being replaced by other regional systems for the most severe errors. PhD-student Alma Mulac has now, all of a sudden, a research topic at the top of the political agenda.
