  In this paper, we interrogate the current views on medical expertise, and expertise more in general, by building upon the study of an innovative medical procedure called transcatheter aortic valve implantation (TAVI). We find that phenomena like TAVI require that we modify our traditional views of expertise to acknowledge its social, material and distributed nature. We also find that in the case of TAVI expertise feeds upon the broad circuits of knowledge created by the combination of professional relationships, social ties and, increasingly, economic interests. Becoming and remaining an expert implies not only being socialised in a local regime of activity but increasingly also participating in, learning to navigate, and exploiting alternative and potentially competing circuits of knowledge, which may be controlled by private companies. The case of TAVI helps us to appreciate expertise as a translocal and connected phenomenon and foreground some of the implications of the emergence of proprietary circuits of knowledge.

  Digitalisation is usually about process innovation with the use of IT, i.e. automating or informating organisational processes. However, redesigned processes are often misaligned with the underlying digital infrastructure. For instance, improving patient logistics with the help of IT is a key aim for current e-health initiatives, but has proven to be quite challenging in practice, and is sparsely dealt with in the literature. Our research question is, how can a process innovation initiative successfully interact with an underlying digital infrastructure? Our empirical evidence is an in-depth case study at a new high-tech hospital in Norway. Building on a proposed framework of interaction between process innovation and digital infrastructure, we identify and analyse two governance and two architectural mechanisms. Theoretically, we contribute to the digital infrastructure research by proposing a configuration for successful process innovation, in a complex e-health context. For practitioners, we show that lightweight IT can serve as a mediating technology in the configuration.

  In this paper we investigate tensions in large e-health infrastructure programs. Currently, there is a gap between the high expectations to e-health and the fragmented status of IT services. In response to this situation the health authorities in many countries has called for a strategy of central IT governance and enterprise architecture, but many large programs experience tensions. Our research question is, how can we understand and manage the tensions of large-scale programs in e-health? The empirical evidence is a multilevel study of a national e-health Norway initiative, at three levels; national, regional and project. We find that the governance-architecture approach is not effective in balancing the tensions of e-health infrastructures. Our proposed solution is to differentiate between stable and unstable elements of the infrastructure; the stable elements can, and should, be managed top-down, while the unstable elements should be subject to local innovation and decentralised governance.

  Managing the development and implementation of large health information infrastructures has proved to be quite difficult in many countries. The root of the problem is managing complexity in unstable environments. In this study we investigate a large case in Norway, an ePrescription project, using a real option analysis. We offer three conclusions. First, the real option analysis is a valuable tool for analyzing not only risk in IT projects, but also for understanding the complexity issues of large health infrastructures. Second, we find that in large public health IT projects, the pressures for increased complexity are quite strong, particularly when it comes to including more stakeholders in the solutions. These pressures are well-intended, but results in choosing the options that increase the risks of the projects. Third, in such projects the forces fighting for the options of lower complexity are hard to see, and have difficulties in voicing their opinion.

  This paper describes the open-ended, exploratory and evolutionary development of a communication infrastructure, through an empirical case study of the introduction and utilisation of telemedicine technology in a hospital department. Developing and using a communication infrastructure necessarily require partners, and in order to enrol the partners and build a network, detours were necessary. In the case the detours mainly consisted of staging individual transmission events (¿stunts¿) in cooperation with prospective partners. These stunts provided a concrete opportunity for cooperation, and although the stunts were singular and specific, knowledge and experience from these events also had general value (spillover). Thus, far from being just a secondary and non-productive activity, the stunts provided the core learning situations in the exploratory development project.

  In this paper we investigate the IT systems silo problem in e-health infrastructures. After three dec- ades of user oriented systems development the health sector is characterized by system diversity and fragmentation. In response to this situation health authorities in the EU countries have called for a strategy of standardisation and integration; aiming to reduce complexity. It is assumed that this strat- egy will lead to less costly (through better co-ordination) and better health services (through more reliable data). Our research question is, how can we understand and manage the socio-technical complexity of large-scale integration in e-health? Our empirical evidence is a large e-health programme taken by the Southeast Regional Health Au- thorities in Norway, in order to reduce the number of systems and user environments. In particular we investigated the implementation and integration of an electronic patient journal system for 12.000 us- ers. Considering our findings, and building on infrastructure and complexity theory, we find that the current approaches to resolve silo integration are relatively effective in reducing organisational com- plexity, but they may increase overall complexity over time.

  This paper explores the role of enterprise platforms in the context of pervasive enterprise-wide processes of digital transformation. The paper empirically illustrates and theorizes on how digital transformation processes are challenged by inertia rooted in existing historical layers of technical and social arrangements. Empirically, we report from a longitudinal case study of a global oil and gas company following their various attempts at digitally transforming their document management practices and platforms. After prolonged efforts to facilitate transformation with an enterprise platform they eventually succeed with digital transformation after nearly six years. In making sense of this, we build on path theory and assemblage theory and theorize digital transformation as a path dependent process, where new paths can be created through stabilizing new assemblages and de-destabilizing others. The paper contributes by theorizing the role of inertia in digital transformation processes and the role of enterprise platforms. We conclude by giving insights on how practitioners can manage path dependency in digital transformation processes

  Designing and developing technologies used to support distributed cooperation across organisational boundaries is challenging. Traditional design strategies do not appear to be suited to the situation at hand, as we will have to deal with vague visions rather than defined requirements, and most importantly, the development has to happen in a complex setting with a multitude of stakeholders, over whom no single actor has control. This paper describes the exploratory and iterative development of a telemedicine infrastructure in a hospital department. The need to enrol partners for development as well as for use required detours from the original goals. In this case the detours took the form of stunts, individual transmission events that were not parts of a grand plan, but which emerged in an ad hoc manner. Even if the main objective of the stunts was on an immediate nature, related to their successful accomplishment, the experience provided spillover knowledge that accumulated and proved valuable. Thus, far from being just a secondary and non-productive activity, the stunts provided the core learning situations on the project.
